Call My Aged Care (1800 200 422) or apply online Check your assessment eligibility | My Aged Care Provide your details & complete a short screening

Assessment
A government assessor visits you at home They review your health, independence, risks & goals You’re given a classification (1–8) or a short-term pathway (e.g., restorative or end-of-life care)

Outcome & Prioritisation
You receive a letter confirming eligibility If approved, you’re placed in the prioritisation system If not approved, you’ll be given reasons & options

Choose a Provider
Use My Aged Care. Find a provider tool Compare services & pricing Give them your referral details

Sign Agreement
Review the Support at Home service agreement Sign with your chosen provider to start services

Add-ons (if needed)
Assistive Technology & Home Modifications (up to funding limits, e.g. $15,000+) End-of-Life Pathway (short-term higher funding to remain at home)

Review & Reassessment
If your needs change, request reassessment Budgets & services can be adjusted

Transition from Home Care Package (HCP)
f you already have an HCP, you’ll automatically move across on 1 Nov 2025 Funding level & unspent funds carry over Your provider will update your agreement
